What is a Derm PA?

Derm PAs are licensed healthcare providers who work under the supervision of a Dermatologist and deliver healthcare based on a "Physician/PA Team" approach. Derm PAs diagnosis, evaluate and treat a variety of skin conditions, including surgical and cosmetic procedures. The medical and surgical services provided by the PA are delegated by their supervising physician based upon the experience, knowledge and training of the PA. Most Derm PAs work autonomously in their practice, but always with the supervision of a dermatologist.

Derm PA Training

Physician Assistants are trained in programs accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA). The average program is 27 months with courses in basic & behavioral sciences and clinical medicine and a total of more than 2,000 hours of clinical rotations in family medicine, internal medicine, obstetrics & gynecology, pediatrics, general surgery, emergency medicine, psychiatry. After graduation, PA's are required to pass the Physician Assistants National Certifying Exam (PANCE), administered by the National Commission on the Certification of Physician Assistants before they can practice. In order to maintain certification, PAs must complete 100 hours of continuing medical education hours every two years and take a re-certification exam every 10 years.  Derm PAs may obtain additional training through educational resources such as CME conferences, dermatology journals, dermatology grand rounds, self study courses, and the SDPA Diplomate Fellowship.  Physician Assistants often provide lectures at PA programs and serve as clinical preceptors to PA students.

Medicare pays for medical & surgical services by PAs at 85% of the physician fee schedule in all practice settings (with the exception of "incident to").  Here are the Medicare policies for billing services provided by PAs (courtesy of AAPA):

"Incident to"-a Medicare provision that allows services provided by a PA in the OFFICE to be billed under the physician NPI with 100% reimbursement

"Shared visit Billing"-a Medicare provision that allows services provided by both the PA & the physician in the HOSPITAL to be billed under the physician NPI with 100% reimbursement

CMS Electronic Health Record Incentive Program and PAs- PAs are not eligible for the Medicare EHR incentive program, but are in some instances, eligible for the Medicais EHR incentive plan

Medicare enrollment for PAs- PAs must apply for first for an NPI# before enrollment in Medicare by the Provider Enrollment, Chain & Ownership System (PECOS)


Medicaid is a program funded by both federal & state governments that provides medical assistance to low income individuals. The federal government pays 50-80% of the cost of Medicaid, but it is the individual states that administer the program, decide who can provide medical care, and provide the reimbursement.  In contract to Medicare, each state determines whether PAs are eligible providers under its program.  All states cover PAs in the fee-for-service or managed care programs at the same or lower rate as physicians.  PAs need to check with their state Medicaid program to obtain the details on their state's rules.
